Vaccine Requirements

Dear Parent or Guardian:

We are contacting you to make sure your child, who is enrolled in the Academies @ Roxborough, is ready to attend school during the 2022-2023 school year:

If your child is in the 9th grade: The state of Pennsylvania mandates that ALL 9th graders receive  the meningitis conjugate and the Tdap vaccines. They should have received these shots between the ages of 11 and 12 years.

If your child is in 12th grade: ALL students entering the 12th grade must receive their second  dose of the meningitis conjugate vaccine.

If your child has not received these vaccines, please make an appointment for your child to get  vaccinated before the start of the new school year.

If your child is in the 11th grade: The state of Pennsylvania mandates that ALL 11th graders  receive a physical exam (MEH-1 Form).

If your child needs to take prescription or over the counter medicine in school, please have the  doctor fill out the Request for Medication Administration (MED-1) form.

If you would like your child to be tested for COVID-19 in school, please, fill out the Consent for  COVID-19 testing as we get closer to the start of the new school year.

ALL students must have a Student Emergency/Medical Information form (S-865) filled out for  the 2022-2023 School year.

All forms are available in the Nurse’s Corner on the Academies @ Roxborough website. Please  fax or bring the forms to the Academies @ Roxborough addressed to: Attention School Nurse
